SECOND REQUEST FOR PRICE QUOTATION

The PDIC invites you to submit a quotation for the items stated below:

Description Approved Budget of Contract (inclusive of all applicable taxes)
2 pcs. Wristwatch (Female watch; Automatic)
2 pcs. Wristwatch (Male watch; Automatic)
5 pcs. Plaque, Etched and Curved Glass - 2 pc. Leaf Design, 8" total height
(please see attached design) 		P20,000.00 (P10,000.00/pc.)
P20,000.00 (P10,000.00/pc.)
P15,000.00 (P3,000.00/pc.)

Total ABC - P55,000.00

Eligibility Requirements:

  1. Current/valid DTI/SEC Registration (certified true copy);
  2. Current/valid Mayor's/Business Permit (certified true copy); and
  3. PHILGEPS Registration Certificate (certified true copy) or
    PHILGEPS registration confirmation thru email (hard copy)

Terms and conditions:

1. Terms of payment: Payment will be made after the delivery of items through check. There is a penalty of 1/10 of 1% for every day of delay.
2. Period of delivery:
1) For wristwatch, ten (10) working days after receipt of approved Purchase Order (P.O.); and
2) For Plaque, Etched and Curved Glass, twenty-two (22) working days after receipt of approved sample.
